I agree with what you are saying but more specifically what is the
difference between Change Level - Review and Change Level IA -Review and
Change Level CI - Review.
What is the difference between the Default Approval Phase field and the
Process Name field?

I believe that these are used for the multiple approval processes that
a change can go through. If I remember correctly, there are 4 points
in the lifecycle of a change that an approval is possible.
Pages 37/38 of the change users guide covers the various approvals:
- review approval
- business approval
- implementation approval
- close down approval
