First two things to check, Rick, did you tell your import to "Import In Place" or "Replace Objects on the Destination Server"?
Second thing, I am guessing there are two things it doesn't like about your Status field--as if you have changed two qualities, perhaps your permissions and attributes. Have you ever exported your definition files, mucked with the definition files as text, and reimported them? It might not have given you an error at the time, but that could cause it to blow up on the new server. Since it's throwing up all over your Request ID field, you have some sort of ick that is messing up all your fields. Perhaps copy the form, make the copy as generic as possible, export the form, then try to import it. This may be a situation where it's just easier to recreate the form on the new server. How many forms is this happening on, and have any of your imports succeeded?
