On the Definitions tab of the form you should see the name of the Audited From 
Form: in the Audit panel.

Hmmm....if you have relationships turned on on the server, the form that's 
auditing TO the audit form should show up as a relationship....or if you have 
an ARInside of the server, that should show it too....
