ITIL aligned implies (ITIL is a best practice not a standard or a rule)
that the custom application is a component of the business process
architecture of the enterprise architecture and that the application has
been defined by a detailed process flow chart and is needed in support
of an organizational business objective or governance mandate.

Making a custom application ITIL compliant just means understanding the
best practices and putting as many into play as possible as defined by
the environment.  The programming is no more difficult than any other
custom application.  It is exactly what Remedy (BMC) has done.  If
you've decided to go the custom application route, the cost may be
increased a bit due to the necessity of hiring a developer or project
manager well-versed in ITIL best practices.  I don't think there is any
particular magic to it.
