Thanks for the quick response.  I found a way to do it.  Not sure if
it's "pretty" but it works.  I didn't think to just map the fields from
the incident form over to the dialog so the dialog could reference them
in the "on close" map.

Working as intended though.

Michael,

Use temp fields in the "on close" map.
Then add an extra Set Field action (after the open dialog action) that
will do the appending. (And you can choose to clean up the temp fields
too in another Set Field action.)
