Melissa, I had a similar issue - I think there's a separate install that you
need to do for the CAI plugin, and/or a configuration setting that needs to
be in place for it to be enabled.  I don't remember all the particulars off
the top of my head, but I think there was a KB that covered this.

This is a new install.  We just yesterday installed 7.0.02 patch 4 hoping
that it would resolve the issue, but it hasn't.
