I know that it's rather tedious, but I would try turning on the workflow logs and submitting a request that ends up in the Console, and then track through each workflow object that managed data to/from each of their related forms to ensure that I fully understand the entire process and that I understand each of the places that the fields already in the Console are populated from. Then I could ensure that my new field IDs track accordingly.
The new ITSM subsystems like this one are so new and so convoluted that this is often the only way to get it right without hacking one's way through the dense underbrush of code.
