I've been through the painful process of getting this to work. 

Just a few thoughts here.

1. I see that your field names have special characters in them. 
(asterisk*). Is that their true Database value?
Try to submit without using the field names and use only the !ID!.

example: replace
Summary*+!301544700!: test request
Urgency*!1000000162!: 3-Medium
with
!301544700!: test request
!1000000162!: 3-Medium

Here is an example of what I use. Notice that my field names do not have 
an special characters before the !fieldID!
Schema: ****
Server: ****
Login: Mail ****
Password: ****
Action: Submit
Format: Short
Submitter's Group !536870980!: Systems Services
Submitter's Division !536870979!: System Production Services
Submitter !2!: Mail User

2. I am not !00%  sure about this but try using Format: Full..... to see 
if the error message returns will more detail, like some other required 
field is missing a value.   "The full format lists the information for all 
accessible fields, with each entry separated by a line of hyphens.  The 
short format returns only the fields defined in the results list. If not 
fields defined for the results list it returns the short description 
field."

3. Also, remember all required fields for this form need to be supplied in 
the email you are constructing.

I want registrate request from email system to ARS.
I write and send next message to AREmail inbox(Subject:

Email engine reply me whith empty message, subj "RE
:test_email_rqc_request", but no requst's registred in ARS.

Form "AR System Email Error Log" don't have any error's 

if field "Summary", for example, empty in message - i get error 4946 and
replied message have next text:
