I can confirm Thad Esser's finding that the you have to leave the
Escalation thread running (Disable Escalations NOT selected) while the
server is in Development Cache Mode in order for the CMDB 2.1.0
installer to complete a new installation successfully.  Apparently it
took BMC two weeks to figure that out, and it is still undocumented in
their KB and release notes.

BTW, I still see no sign of the CMDB 2.1.0 Patch 001 on SupportWeb in
spite of the release of a Technical Bulletin about it on 15 November.
Maybe they delayed the release to fix the installer - I already had a
defect logged against the 2.1.0 upgrade installer that was documented as
included in the patch.
