I did look at the arplugin log (finest). The arpluin log shows the last log line as: <REMEDY.ARF.CAI> <FINE> Entering ARPluginCreateInstance while arplugin was still alive. There should be a line for <REMEDY.ARF.CAI> Leaving ARPluginCreateInstance. I think it's REMEDY.ARF.CAI that causes the problem but still don't know why it does.
