Hey Mike - not sure which version you are on, but in 7.x Change the notifications OOB are included on the CTM:People form in the notification tab. So the only reason someone might NOT get notified is if someone has updated the notifications to preclude that for someone, OR if that person is set to something other than email as the notification method (like None), OR if you might be on a patch level that is broken. To my knowledge, the notification should go to the approver regardless of process flow, but I've been known to be wrong before. I would recommend you check the notification tab for the person in question first. You might find the problem right there. :)
