Andy, I really wish there was a way to tell what version of the ARS object(s) the User tool/Mid-Tier is using. ( Like even showing the "Last Modified Date" for the form would be helpful.) I suspect that the reason your first test "failed" is that the client did not in fact have the most recent version and was instead using a local cached copy of the object that was a previous version of the active link.
I hate to say it, but there are times that I really wish, for development purposes, that I could tell the ARS client (User Tool , or Mid-Tier) that it should always to get the objects a fresh and NOT use any local cache. When testing/developing caches are often a blind spot.
