I have seen that in Citrix as well but this is a non-Citrix environment.

Sometimes on citrix sessions, we have seen those issues.

You can see "user currently connected from other machine..user has over-ride 
option" in arerror.log

That makes sense unless you have a user
who every time they login receives the message that they are currently logged
into another computer even though they are not. Has anyone had this experience?
