With HD 5.6 when I once had that error and after checking all the checks you mention (use of underscore etc) and encountering the same problem as you, I had contacted Remedy support and it turned out to be a bad report file. They had sent me a patched file and I had to delete the original file from the Reports form and attach the new one they had given and it had worked for me..
In my case it had not worked on any of my client PC's so it was a global problem. If thats the same case with you, you might want to check with Remedy support and see if they can dig up a patched file for you. That you are on an older version might be an issue as I think 5.6 is way out of support but it never hurts to try..
