We found the solution with the help of BMC Support. Apparently, there is a bug in the installer when you perform an upgrade from 7.0 to 7.1 for the Approval Engine.
The filters used in AP:Notification have changed from using the "Instance ID" which in 7.0 was linked to the name of the approval, to the actual Instance ID in 7.1. Since the filters were not updated, it didn't work properly. The solution was to export the 15 records in AP:Notification and reimport them. That regenerated the filters and solved the problem.
