Have you restarted Tomcat after flushing the cache? If you restart Tomcat and run performance monitor, you should see a high level of activity for a few minutes while the cache is rebuilt. If you do not restart Tomcat, the chache for each form is only built when it is accessed for the first time (the same as if you have no prefetch configured).
