Ok...on that one web server I would recommend removing the server in question (from the mid-tier config page), then re-add it with the same information you did for the others...then re-check

One of them gives the error ARERR 623 when we try to access it. This is typically an error shown when you have the wrong mid-tier password. In this case though we have re-set the password in the Server, bounced hte server, and re-set the password in all 4 mid-tier servers. Just that same one has the problem. Ideas? I'm stumped.
