Martin, It depends what you want to 'health check'. The first thing to do is set the sticky flag on the F5. The next thing to do is find something for the F5 to request that'll ascertain whether the web server (i.e. Tomcat) is alive - perhaps could make a request to a static file in the Midtier, such as a small image? You may then want to go further and configure something to check the Midtier itself, and I'm not sure what to check at that point (as essentially, you really want to test the link betwern MT and AR System).
