Please make sure you would have a latest patch applied for your mid-tier. If End user is accessing URL from firefox, you need to make sure allow port 8080 in your firewall if you have any. We also got the same issue for some our 100+ users out of 2000 users who are accessing through firefox. After opening port 8080, the issue got resolved.
