We have also seen this issue, no matter what midtier settings we chose and 
how many times we flush the cache midtier still did not recognize the 
changes.
Later we found that after flushing the midtier cache you go to your client 
browser's temporary internet files folder and delete the files there. Login 
to midtier again and we are able to see the changes. This has been very 
consistent for us.
For IE it is C:\Documents and Settings\spatchip\Local Settings\Temporary 
Internet Files folder.
The key is go to the folder manually and delete all the files, if you just 
click delete  in your internet explorer options, it did not always worked.
