This is quite simple actually and can be done with just a minor customization.
Compare the database and the transactional value of that assignment field whatever it is, and if the DB. and TR. values do not equal each other, AND the current value (not TR or DB - just the field with no TR. or DB. prefix) is not null, send that notification to whoever you wish to send it to.
