Clear the server cache and also clear the browser cache & cookies. Are you using IE to access the Mid-tier? Are you using SSL? If yes, on IE, under Tools > Internet Options > Content, "Clear the SSL State".
Under File, select "New Session". In the new browser, try you URL again.
