Just a point David, I hope you didn't edit the system xhtml11.conf?

If you did it will get overwritten when you upgrade your asciidoc, as
Dan said you should use a *local* copy, and you only need it to
contain the relevant sections defined by the [something] not the whole
file.  If you copy the whole file you will miss out on
corrections/additions when you upgrade asciidoc, and even the old conf
might not be compatible with a major new version.
