Since I'm using -d book in the toolchain docbook45.conf is where the 
changes must occur.

I've uploaded the patch I'm using.

Since docbook45.conf can potentially be overwritten every time hg -v pull 
&& hg -v update is issued...

What's the suggested way to keep these changes?

Say for example I place the changes into a file docbook45-lean-tables.conf 
and my toolchain is normally...

a2x -d book -f xhtml

What's to correct toolchain to reference the docbook45-lean-tables.conf 
configuration file?
