Well, the generated file is html4, not xhtml11 thats why it has no toc
(which needs javascript) and everything is ugly because of no css.

Possibly the installation failed, I don't know anything about freebsd
packages though.

Can you run asciidoc --version just to check its getting the right version.

Can you run with -v and post the stdout & stderr outputs?

Also does it work correctly if you add the -b xhtml11 option?
