The standard xhtml11.conf does not include html4.conf see
https://code.google.com/p/asciidoc/source/browse/xhtml11.conf so it is
being modified somewhere, probably accidentally.

PS even if you copy the file from the repository I'd be wary about what
else may have been changed, perhaps install from the asciidoc website
repository or tarball.
