Here is asciidoc 8.6.7-1, fop 1:1.1.dfsg-2ubuntu1, docbook-xsl 
1.78.1+dfsg-1, evince 3.10.0-0ubuntu2 on Ubuntu 13.10 64-bit.

I think it's *not* a problem with the PDF viewer, but just to be sure, 
you'll find attached a PDF showing the bug. Can you post yours ?

Asciidoc does fiddle somehow with the docbook XSL stylesheet, as visible in 
/etc/asciidoc/docbook-xsl/ .
Next step would be that I test that in a pure docbook-xsl setup unrelated 
to asciidoc.
