Yes, yours is crooked, mines not.  That suggests its the version of
fop or docbook-xsl.

For fop fo.xsl and common.xsl are all that are used. They mostly set
parameters and tell docbook-xsl what to put in the toc (see the end of
common.xsl), but nothing about the layout, thats entirely docbook-xsl
and fop.

You need the instructions above for toc generation IIUC, so I don't
know how that will go.
