You can't do this on shared hosting account, you only rent a part of the machine, and can't install an application or service.
If your hosting company supports SQL Server (most .NET hosts do) they will be happy to create a database for you (usually on another machine) and provide you with the server info and credentials needed to access the database. Usually they charge a little for this service, or you may have to upgrade your account to one that includes this service. Once you have access to the database, you can add tables, etc. using enterprise manager, or even a MS Access Data Access Project.
