The book should refer to the operand by the name given the syntax diagram, not 
the English name "second operand".  I have talked to several of my assembler 
comrades and they agree: Using the phraseology of "second operand" is not 
specific and leads the reader to counting from left to right to the second 
operand.  When I look at the PDF book, the subscripts are tiny and hard to read 
with these old eyes.  I would have to put on my glasses to actually be sure of 
the subscript.

I understand now but it was, and still is, confusing.

It has always been thus. The reason for the subscript is to denote which operand
is being discussed--since storage operands are routinely broken up into pieces
e.g., B2 and D2.
