Everything looks good in your checklist so I thought I'd try the netfarm port 
to see if that helps - thanks Paul and marrco.
I disabled the existing 'Run Clamd' service first.

There aren't any instructions so I extracted the clamav-win32-0.93.1 directory 
and the Microsoft.VC80.CRT subdirectory then ran freshclam, copied in 
clamd.conf from my old ClamAV install and modified:
LogFile "C:\Program Files\clamav-win32-0.93.1\clamd.log"

I am running clamd manually from a command prompt now and netstat -an|find 
":3310" shows:
TCP    0.0.0.0:3310           0.0.0.0:0              LISTENING

No errors in clamd.log, no log at all for freshclam.

Sending another test, it's still not getting detected.  I must be missing 
something.
The directory structure is supposed to be:
C:\Program Files\clamav-win32-0.93.1 - all clamav files
and
C:\Program Files\clamav-win32-0.93.1\Microsoft.VC80.CRT - all MS DLLs
right?

First of all, I hope you installed the "daemonized" version of ClamAV
(I mean http://w32.clamav.net/) and not "ClamWin"; next ensure your
clamd is up and running and fire up a "netstat -an|find ":3310" if all
ok
you should see port 3310 in "LISTENING" state; next, just to ensure
all is ok on the clamd side, move to the ClamAV install folder and have
a look at the "clamd.log" and "freshclam.log" files to check if you spot
any errors; if not, then we can assume clamav is ok, so move to ASSP
and tick the "scan" options for "whitelisted", "no processing" and
"local"
confirm the changes and then retry sending the EICAR test file
