They exist but are exceedingly rare (hence the "for some unfathomable
reason" in my original post!)

Doesn't that describe at least 80% of phone calls? I know that here
in the US 99.99% of our customers have POTS analog telephone lines.
The only ones who don't are commercial accounts that have T1 or PRI
service, and maybe one guy who has a BRI for some unfathomable
reason.
