I am not sure if this related to your issue, but MixMonitor would not
work properly for me on version 1.4.18. I suggest going to 1.4.19 and
see if you get better results, it cleared my issues up when I went to
the .19 version.

According the docs, all you need to do is set:
monitor-format = wav
monitor-type = MixMonitor
And that should be it but it seems to have no effect and I don't know
where to go from here?
