Figure out what is different between this user and the other users.
Whatever that difference is, that's where I'd start. It could be
hardware, wiring, network, software, PC specs, NIC, viruses,
application, teenagers (bittorrent), etc.

being able to have a conversation is the priority. You simply can't when there
is a 10 second delay.
