Figure out what is different between this user and the other users. Whatever that difference is, that's where I'd start. It could be hardware, wiring, network, software, PC specs, NIC, viruses, application, teenagers (bittorrent), etc.

Basically, I don't want that huge delay, quality is of course important but 
being able to have a conversation is the priority. You simply can't when there 
is a 10 second delay.

Can anyone make suggestions as to what might fix this issue?
