I have unlimited incoming calls on my cell phone plan (Fido).
I have configured Asterisk via Freepbx to call me back based on my cell's CID
It never actually answers the phone(I get a re-order tone) but after "n" number 
of seconds it
calls me back and gives me dial tone via DISA from there I can dial any number 
without incurring any cell
phone charges. Later versions of Freepbx allow calls to be dropped and return 
dial tone to make another
call etc.
The plan cost about $48 per month
