1. I do not think you can get a opensource version.

2. The process should be client side and it is possible. If the CRM allow
customization and CRM user using Windows OS and IE as Operating system use a
SoftPhone ActvieX Control to achive this target.

Simply Insert a Custom Text field and couple of buttons ( Answer, Call ,
Transfer and  Hold). You have full control of what is happening on each
call.

I never implement this kid of a System for a CRM user using a Linux or Mac
workstation. Should be the same way.
