Very close to a set-top box is a mini computer + HD camera + Eyebeam or similar software and a Windows OS with nothing but the Eyebeam software.
Another option is a phone like GXV3000 - which I just picked up yesterday - that has a video output jack. I haven't tested it yet so I am not sure about the quality. Comes with built in camera but has USB ports.
