Thanks... you just solved my problem. To document in case someone else does the same thing.
Using Yealink T46G (nice phone BTW)... set it up, working perfectly. Decided to try the OpenVPN client, hooked it up first try, worked perfectly... Played around with settings on the PBX... Played around with settings for the firewall... This was the mistake, set the PBX from Public IP to Private... Tested the phone... in/out calls worked perfectly. But when the phone system tried to "Follow Me" to an external number, it did not work. I have tested and it seems to work now... Try running "rtp set debug on" at the Asterisk CLI, and see where it's sending your RTP packets for these calls. If you see it sending packets, but not receiving any, you'll know it's an inbound FW issue, and you likely need to forward the correct RTP ports. If you see it sending somewhere it shouldn't be, then there's a problem with the sip.conf settings somewhere - externip, externhost, localnet, etc. - check those. As Brian mentioned, this is typically a firewall issue. Since the source and destination for RTP traffic are both external, lots of firewalls can't handle this case where they can send calls that originate within the network just fine.
