my guess your missing this one special important step Eliminating Audio and DTMF Problems. You can avoid one-way audio on calls and touchtones that don't work with these simple settings in FreePBX: Settings -> Asterisk SIP Settings. Just plug in your public IP address and your private IP subnet. Then set ULAW as the only Audio Codec. If you see it sending packets, but not receiving any, you'll know it's an inbound FW issue, and you likely need to forward the correct RTP ports. As Brian mentioned, this is typically a firewall issue. Since the source and destination for RTP traffic are both external, lots of firewalls can't handle this case where they can send calls that originate within the network just fine.
