NAT issue... only showed itself when doing "Follow Me".

Eliminating Audio and DTMF Problems. You can avoid one-way audio on calls and 
touchtones that don't work with these simple settings in FreePBX: Settings -> 
Asterisk SIP Settings. Just plug in your public IP address and your private IP 
subnet. Then set ULAW as the only Audio Codec.

but not receiving any, you'll know it's an inbound FW issue, and you likely

As Brian mentioned, this is typically a firewall issue. Since the source
and destination for RTP traffic are both external, lots of firewalls can't
handle this case where they can send calls that originate within the
network just fine. You've posted your FW configuration, but are those ports
also forwarded/NATed to the correct IP?

If I turn Following Me on and forward to a cell phone and land line... the
above two work as expected, extension works, and the cell and land line
ring. But if I answer the cell or land line, there is no audio.****
