That did the trick:

gunzip -c astlinux-0.6.1-net5501.img.gz | dd of=/dev/sdb bs=16k

And then it took me a little time to figure out how to setup the rest of the CF as a partition for UnionFS. I guess I should put my notes into a cookbook form for the next person to use.

Now it will be down to porting my old hand-rolled Asterisk v1.2 configuration files into the scheme Asterisk v1.4 and AstLinux like...

This *includes* runnix and is designed to be written to a CF card. The
ISO image is designed as a bootable CDROM, which isn't what you want.

Once you have your CF card installed with runnix, you will only need to
download
http://www.djhsolutions.com/astlinux/astlinux-0.6.x-net5501.tar.gz in
the future.  This will contain the .run files needed.
