Just in case anyone else experiences the same problem provisioning a new box, I 
am 99.9% sure my original issue (no eth0 or any interfaces besides lo, after 
1st reboot during setup sequence) was due to a bad CF card. One of the things I 
did to finally get the net4801 properly built was to use a different CF. 

Just for the hell of it, I tried to install to an alix box last night using my 
"original" CF from the 4801 installation (of course w/ an alix image) and I had 
the exact same problem of only having an lo interface after the 1st reboot. 
Using a different CF card fixed the problem again.

You need to add:

BRIDGE0="eth1 eth2 eth3 eth4"

to your /mnt/kd/rc.conf.d/user.conf file via the Network tab...
