I briefly tested the scenario you describe and if worked for me as
expected, i.e, RefTeX scanned the .bib database anew and found the entry
I've just added or changed.

Stab in the dark: Are you using any kind of Minibuffer enhancing package
like vertico etc.?  If not, I suggest you come up with a recipe starting
with "emacs -Q".
