Apologies for not dropping in on this thread to help any sooner than I have done. The way prices work for gold, on my end at least, is that it shows two prices, in your example it would be something like $39.99 $0.00 - This would mean that when you check out it'll be free. Also be sure that you are buying the s1 ultra edition addon and not a different one by mistake.
It should be free for the entire month of January, so you have a little while to sort things out.
As far as instructions go, to get narrator activated, if you have a kinect you can use the voice command, or if you don't, press the Xbox button and hold it. When the controler vibrates, let the button go and press menu (the button where start should be) and you should hear "starting narrator.
From there, just have a play around with things and see how you get on, I'll be happy to help if I can.
