A friend of mine who does aviation medicals rang me up one day as he had a
pilot in for a medical and he had no idea of the local radio frequencies -
He was an older pilot and he was a little concerned about his switched on
ness as a pilot!!!

I'm thinking more of the threads in re.aviation.soaring of US pilots who
have failed their PPL medicals, so they get their 'glider rating' after
a few flights, and then buy a motor glider, with a self declaration of
fitness to fly.
I'm also thinking about elderly instructors who are subsequently fond to
have medical problems that prevent them flying, but have been quite
happily self declaring their fitness to fly, perhaps from ignorance,
perhaps intentionally.

You mean the entire British Gliding Association? Who fought for years
against any medicals on thwe grounds that it was a lot of money for
little
to no demonstrable safety increase backed by statistics on their own
accidents. Most glider accidents are caused by brain failure it seems
which
doesn't get picked up in any medicals.
