p. 312: This is very tiny, but it looks like there's a space missing
between dialog(figure 260), in the middle of the page.
p. 314: The cross-ref to Figure 255 should refer to figure 262.
p. 318: The cross-ref to Figure 242 is incorrect. I can't figure out what
figure it should refer to.
p. 319: The image for figure 267 is missing.
p. 320: The cross-ref to figure 241 should refer to figure 268.
p. 321: The cross-ref to figure 240 should refer to figure 269.
(Ch. 11)
p. 325: The cross-ref to figure 270 should refer to figure 272.
p. 334: The cross-ref to figure 279 should refer to figure 281 (this happens
twice on this page).
(Ch. 12)
p. 343: There's a little screenshot in the middle without a caption or #. As
before, this is probably intentional.
