[authors] Question on Reviewing. New Author!!

How do you prefer the reviews to be written. Do you make changes to the 
actual document or do you want a separate document with the changes and 
ideas for the areas we have suggestions with.

Also if we make changes to the original text do we make it a different 
font or otherwise distinguish it from the original text?
