[authors] Trouble uploading a reviewed document

I've finished a review of the document "Upgrading from 1.1.x". I've tried a couple of times to upload it but it hasn't seemed to work. I get a message that says "Please correct the indicated errors."  But no errors seem to be indicated. Please help! Thanks.   Dick, Webster, NY
