One unresolved item: I was unable to locate the Setup.exe file that was referenced in the text. A file search came up negative. A note was inserted concerning this.
I think that depends on the operating system, in which case a note is definitely in order.
Another item: Is it customary to have the changes of a prior review still in a published chapter as in this one?
No, although for awhile I was doing that for the benefit of the translators.
