Yes, it is there, but it is marked "Visible" not "Published" so
perhaps you cannot see it. Here is a direct link:

I have sent it offlist as requested. Note that Appendix A is
BEFORE Ch17, Macros, so it needs to be moved to the end (use the
Navigator). There are other problems, as mentioned in my previous
note. I hope you will be able to fix it without too much difficulty.
