The Screenshot-16 shows you the table of contents created with
OOoDefinitionTerm set to level 1. Notice it is in the ToC, but not in
the navigator. Also, notice it is not hyperlinked. I edited the ToC and
choose All to make sure that it was applied, but that feature isn't
working either.
I downloaded a fresh ott and created the example file from ott.

Okay, now I understand, although I didn't see any screenshots. I was confused
before, because you said it had worked for you in 3.0.
AFAIK additional styles assigned to TOC levels using the Assign Styles page are
not hyperlinked and never have been. A bug report (considered an enhancement
request) was made long ago (March 2002), but still has no target milestone
listed. There are 23 votes for this issue; you might like to add yours.

The Navigator problem is probably associated with this. Lack of hyperlinks in
alphabetic and other indexes is definitely related.
